• Support the care management team with discharge planning • Coordinate efficient transitions for patients from hospital to home or alternative care settings • Assist with operational functions of the department • Work under the general supervision of the Director
🎯 Requirements
• Clinical or administrative experience in the healthcare industry • Associate's Degree is preferred
