📋 Description

• Provide support to the Care Management team coordinating patient care • Work closely as a healthcare team member • Perform complex tasks related to overall team functions and activities • Support activities identified during the initial and ongoing assessment and management of patients • Work under the supervision of the Regional Director of Care Management

🎯 Requirements

• 5 years of experience in a hospital, clinical setting, or healthcare-related field • 3 years of demonstrated outstanding customer service experience • 3 years of clerical and computer experience • 3 years of knowledge of insurance billing and managed care • Some college-level coursework or equivalent education/experience preferred • High School Diploma or GED equivalent, or some college-level coursework preferred, particularly Medical Terminology or Medical Assistant coursework • Medical Terminology or Medical Assistant coursework or equivalent education/experience preferred • CHAA coursework or equivalent education/experience preferred • CHAA certification within 6 months (180 days) of hire if in California • 3 years of EPIC Case Management and ADT Patient Access experience preferred • Full-time availability for 8-hour day shifts
