📋 Description

• Provide individual, family, and group clinical sessions and communicate with referral sources and outpatient providers • Conduct mobile behavioral health and substance use disorder assessments within assigned service areas • Complete biopsychosocial assessments, clinical evaluations, intakes, and reassessments • Assess behavioral, emotional, cognitive, social, environmental, and recovery support needs • Perform ASAM assessments and determine appropriate levels of care • Develop admission, treatment, and clinical recommendations • Complete individualized treatment plans with clients and treatment teams • Coordinate treatment and admission planning with referral sources, hospitals, emergency departments, providers, families, and community partners • Support service planning, clinical care, aftercare planning, referrals, and continuity of care • Facilitate utilization review processes and maintain authorization and continued-stay documentation • Maintain timely, accurate, and compliant documentation • Participate in assigned on-call rotations • Maintain confidentiality and professional discretion • Complete required organizational, licensing, and accreditation training • Perform other duties as assigned

🎯 Requirements

• Required to reside and be licensed in Pennsylvania • One of the stated ASAM-aligned qualification pathways: master's degree with relevant major and master's-level licensure or state certification; master's degree with relevant major while working toward master's-level licensure or state certification; bachelor's-level education with CADC or working toward CADC and at least one year of qualifying clinical experience; associate-level education with CAAC or working toward CAAC and at least two years of qualifying clinical experience; high school diploma or equivalent with CAAC; or current Pennsylvania registered nurse licensure, accredited nursing degree, and one year of qualifying clinical experience • Knowledge of substance use disorder treatment, mental health treatment, and recovery-oriented care • Excellent oral, written, and interpersonal communication skills • Ability to gather, organize, and accurately communicate assessment-related information • Ability to establish and maintain professional relationships with clients, families, referral sources, and community partners • Ability to problem solve by gathering and analyzing information • Ability to recognize crisis situations and communicate concerns appropriately • Working knowledge of behavioral health, substance use disorder treatment, and recovery-oriented care • Working knowledge of federal, state, accreditation, and compliance standards • Ability to maintain confidentiality and professional boundaries • Ability to drive and travel frequently throughout assigned service areas • Ability to occasionally lift or move up to 25 pounds and seldom lift or move up to 50 pounds • Completion of required organizational, licensing, and accreditation training
