• Work face to face with injured workers and medical providers • Facilitate advocacy, progressive medical treatment, and timely return to work • Act as a liaison and provide communication between all involved parties • Manage complex medical conditions, treatment planning, and recovery from illness or injury • Serve as a patient advocate for injured workers, families, and caregivers • Guide and coordinate care throughout the recovery process • Assist injured workers in achieving recovery and autonomy through advocacy, communication, education, resource identification, and service facilitation • Identify appropriate providers and facilities throughout the continuum of services • Ensure resources are used in a timely and cost-effective manner to obtain optimum value for the client and reimbursement source • Maintain a home-based office while traveling frequently
• Must live in or around the Baton Rouge, Lafayette, LA region • Medical/clinical or rehabilitation knowledge and experience • Graduate degree in a health or human services field required, with one of the following certifications: CCM, CDMS, or CRC • RN licensure preferred • Valid driver’s license required • High-speed internet required • Frequent travel required • Ability to work face to face with injured workers and medical providers • Ability to support advocacy, medical treatment planning, communication, education, and service facilitation
