• Provide consultation and management of patients with high-risk and complex dual medical and behavioral diagnoses • Provide psychosocial counseling and support services for the Sharp Community Medical Group population • Facilitate integration and continuity of medical and behavioral patient care • Navigate patients through behavioral health benefits, available programs, Sharp HealthCare system, and community resources • Mobilize patient and family resources to maximize medical and behavioral healthcare benefits and quality of life • Complete psychosocial assessments and identify key clinical issues • Document assessments, consultations, recommendations, interventions, care plans, and patient progress • Consult and communicate with PCPs, behavioral health providers, and multidisciplinary team members • Develop and participate in patient care conferences for difficult or high-risk psychosocial cases • Refer patients and families to hospital and community resources • Facilitate patient transitions across the care continuum • Consult and intervene in crisis situations; assist staff with de-escalation • Coordinate critical incident debriefings and develop nursing education • Advocate for patients and families and report mandatory protective concerns • Develop, implement, evaluate, and revise medical and behavioral health care management plans • Utilize individual, conjoint, and group treatment modalities
• Bachelor's Degree in Social Work or other behavioral health related field • Behavioral Health experience preferred • California BBS Licensed Clinical Social Worker (LCSW) preferred • Theoretical and practical knowledge of psychosocial process • Strong assessment and clinical counseling skills • Excellent oral and written communication skills • Excellent problem-solving skills • Excellent documentation skills • Strong collaborative and negotiation skills • Knowledge of HIPAA, patient privacy, confidentiality, and regulatory requirements • Ability to perform crisis intervention and problem-solving techniques • Sensitivity to cultural and ethnic diversity
