📋 Description

• Identify high risk, high acuity patients who would benefit from care coordination • Develop interdisciplinary care plans with providers and team members • Orient and educate patients and family/support members on the care coordination process • Coordinate care information and requirements with other care providers • Resolve issues affecting care progression and foster peer support and education • Evaluate adherence to and effectiveness of care plans and modify them as needed • Measure intervention effectiveness • Facilitate appointments, scheduling, and completion of tests or procedures • Remove or minimize barriers to adherence to care • Provide clinical care in compliance with SCC policy and the New Mexico Nurse Practice Act when needed • Refer patients to additional providers or resources for specialized needs • Document accurately and promptly in the EMR • Collect and analyze data supporting clinical best practices and SCC Quality programs • Support research studies involving care protocols, care planning, and interventions • Participate in staff meetings, meetings, and seminars • Perform other related duties as assigned

🎯 Requirements

• Minimum 1 year of RN experience within an ambulatory/primary care environment preferred • LPN or Medical Assisting experience may be considered in lieu of the RN experience requirement • Completion of an associate degree in nursing required • Bachelor of nursing preferred • Valid Registered Nurse License in the state of New Mexico or another compact state • Must reside in the local market area of Albuquerque or Santa Fe, New Mexico • Ability to document appropriately and timely in the EMR • Ability to perform clinical care in compliance with SCC policy and the New Mexico Nurse Practice Act
