β’ Identify high risk, high acuity patients who would benefit from care coordination β’ Develop interdisciplinary care plans with providers and team members β’ Orient and educate patients and family/support members on the care coordination process β’ Coordinate care information and requirements with other care providers β’ Resolve issues affecting care progression and foster peer support and education β’ Evaluate adherence to and effectiveness of care plans and modify them as needed β’ Measure intervention effectiveness β’ Facilitate appointments, scheduling, and completion of tests or procedures β’ Remove or minimize barriers to adherence to care β’ Provide clinical care in compliance with SCC policy and the New Mexico Nurse Practice Act when needed β’ Refer patients to additional providers or resources for specialized needs β’ Document accurately and promptly in the EMR β’ Collect and analyze data supporting clinical best practices and SCC Quality programs β’ Support research studies involving care protocols, care planning, and interventions β’ Participate in staff meetings, meetings, and seminars β’ Perform other related duties as assigned
β’ Minimum 1 year of RN experience within an ambulatory/primary care environment preferred β’ LPN or Medical Assisting experience may be considered in lieu of the RN experience requirement β’ Completion of an associate degree in nursing required β’ Bachelor of nursing preferred β’ Valid Registered Nurse License in the state of New Mexico or another compact state β’ Must reside in the local market area of Albuquerque or Santa Fe, New Mexico β’ Ability to document appropriately and timely in the EMR β’ Ability to perform clinical care in compliance with SCC policy and the New Mexico Nurse Practice Act
