📋 Description

• Conduct outbound triage and follow-up calls with individuals interested in or referred for SUD treatment • Assess member needs, explain treatment options, and make informed care recommendations • Coordinate appointments, referrals, and follow-up care, including complex cases • Provide psychoeducation, motivational interviewing, and evidence-based support for recovery goals • Apply knowledge of SUDs, ASAM levels of care, contingency management, MAT, and detox treatments • Provide in-the-moment support, crisis intervention, and safety planning for urgent or high-risk situations • Maintain detailed records in an EHR and other systems • Identify and resolve complex care-disruption issues • Collaborate with care support, provider operations, benefits/billing, and interdisciplinary teams • Participate in staff meetings, case consultations, and ongoing training • Adhere to confidentiality, compliance, and ethical standards • Support and provide coverage for other SUD Clinical Navigators • Use AI tools thoughtfully, apply human judgment to AI outputs, and build AI fluency

🎯 Requirements

• LCSW, LMFT, or LPC mental health licensure required • 2+ years of experience in substance use disorder clinical care • Proficiency in motivational interviewing, triage, ASAM criteria, various levels of care, and SUD best practices • Excellent listening, written, and verbal communication skills • Ability to work effectively with diverse populations and commitment to diversity and inclusion • Ability to provide a high-touch, supportive client experience • Comfortable using computer platforms, learning new systems, and handling technology-related tasks efficiently • Ability to manage multiple tasks and switch contexts efficiently • NPI number or ability to obtain one prior to employment • SUD licensure or certifications preferred • Bilingual preferred • Eligibility to work in the U.S. and visa sponsorship information required during application
