📋 Description

• Conduct telephonic screening, brief intervention, and referral of members to trauma specialty treatment providers • Apply active listening, psychoeducation, and motivational interviewing to understand member needs and support goal setting • Use Judith Herman’s phase-based trauma treatment model to provide stabilization support • Identify appropriate levels of care, facilitate referrals, and coordinate care with Spring Health providers • Provide consultation on mental health concerns and family or relationship issues • Provide in-the-moment support and safety planning for members facing safety risks or crises • Communicate with members and providers by email • Document information, maintain accurate records, perform follow-ups, and track progress in multiple systems including an EHR • Identify and resolve barriers or disruptions to care • Collaborate with care support, provider operations, and benefits/billing teams • Assist other specialty care teams as needed • Participate in staff meetings, case consultations, and trainings • Maintain confidentiality, compliance, ethical standards, and Spring Health policies • Maintain member satisfaction and consistent outreach throughout care • Triage and enroll members into appropriate levels of care • Contact members by phone and email to schedule triages

🎯 Requirements

• Required unrestricted mental health license: LCSW, LMFT, or LPC • Minimum of 2+ years of clinical care experience working with PTSD and complex trauma • Experience with motivational interviewing, diagnostic assessment, and triage • Knowledge of phase-based trauma treatment model and evidence-based practices for PTSD, including Prolonged Exposure, Cognitive Processing Therapy, EMDR, and ART • Excellent listening, written, and verbal communication skills • Experience working with diverse populations • Comfortable using various computer platforms, learning new systems, and completing technology-related tasks efficiently • Ability to multitask and context switch • NPI number or ability to obtain one prior to employment • Trauma-specific licensure or certifications preferred • Bilingual ability preferred
