📋 Description

• Communicate with patients and families about continuum of care services • Meet and communicate with referral sources, including case management, social work, clinics, providers, and SNF/ALF facilities • Coordinate care between continuum of care departments, branch departments, and business development • Review eligibility and patient charts to determine appropriateness for continuum of care services • Review and analyze referrals for home health, hospice, and infusion services • Coordinate service aspects to support smooth transitions from hospital to home care • Communicate patient needs and physician orders to clinicians to establish plans of care • Provide educational information to case managers, physicians, hospital staff, and nursing home staff • Assess patient care needs using hospital records and referral information • Identify educational needs for patients and significant others • Adhere to payer skilled-service and homebound regulations • Ensure pending referral information is timely communicated to nursing supervisors, clinical supervisors, rehabilitation services, and others • Coordinate initiation of care and seek appropriate orders for multidisciplinary care • Reassess patients and records to evaluate care, progress, and trends requiring intervention • Communicate with supervisors regarding managed cases awaiting initiation of care • Assess hazardous or unsafe environmental factors and respond according to policy • Work in a constant state of alertness and safe manner • Support Home Health three days during the normal work week and Hospice every weekend, including urgent hospice referrals, timely admissions, and starting Home Health referrals for Monday

🎯 Requirements

• Experienced nurse with Hospice, Home Health, Liaison, or Case Management experience • Dynamic personality with energy and passion for Hospice and Home Health care • Resilience and ability to solve Home Health and Hospice challenges • Required to obtain licenses for Missouri, Illinois, Oklahoma, and Wisconsin within 6 months of starting • Graduate of an accredited school of nursing or education equivalency for licensing • One year of licensed practical nurse experience • Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association • Licensed Practical Nurse (LPN) license from the Illinois Department of Financial and Professional Regulation • For Missouri, Oklahoma, and Wisconsin: LPN license issued by a compact state • Constant speech, standing, and walking; frequent lifting/carrying and pushing/pulling of 0–25 lbs; frequent sitting, reaching, keyboard use/data entry, and sensory abilities • Occasional lifting/carrying and pushing/pulling of 25–50 lbs; occasional lifting/moving of patients; occasional driving • Rare crawling and running
