📋 Description

• Process and review patient referrals for home health, hospice, and infusion services • Determine referral appropriateness, eligibility, completeness, and required documentation • Coordinate smooth transitions from hospital to home care • Communicate patient needs and physicians' orders to clinicians to establish plans of care • Provide education to case managers, physicians, hospital staff, and nursing home staff regarding qualifying criteria and access to services • Assess patient care needs using hospital records and referral information • Identify educational needs for patients and significant others • Adhere to payer skilled-service and homebound regulations • Ensure pending referral information is communicated timely to nursing, clinical, rehabilitation, and other teams • Coordinate initiation of care and seek appropriate multidisciplinary orders • Reassess patients and records to identify progress, trends, and interventions • Communicate with supervisors regarding managed cases • Assess environmental hazards and respond according to policy • Perform other assigned duties

🎯 Requirements

• LPN licensure in Missouri, Illinois, Wisconsin, or Oklahoma • Graduate of an accredited school of nursing or education equivalency for licensing • One year of licensed practical nurse experience • Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association • Strong clinical judgment • Ability to review referral information for eligibility, appropriateness, and required documentation • Intake, referral coordination, home health, or care-coordination experience preferred • Experience with Epic system preferred • Strong organization, prioritization, communication, and attention to detail • Ability to work independently in a remote environment while collaborating with multidisciplinary teams • Flexibility to provide PRN coverage, including applicable weekend/holiday availability • Ability to meet physical requirements including standing, walking, lifting, patient movement, driving, and keyboard/data entry
