📋 Description

• Provide compassionate, evidence-based mental health care through telehealth • Provide comprehensive biopsychosocial and standardized assessments for potential program enrollment • Develop individualized support plans with patients and establish goals • Provide community service resources and make referrals • Provide enrollment and intake services • Respond to telephone inquiries about mental health care and related services • Conduct safety assessments and planning via telephone or video • Partner with patients to create self-directed, sustainable changes aligned with their values • Collaborate with multidisciplinary team members to develop patient support goals • Participate in multidisciplinary patient care conferences as needed • Document interviews, consultations, referrals, contacts, and supportive services • Complete documentation and reporting according to departmental standards and federal and state regulations • Perform other duties as assigned

🎯 Requirements

• Master's degree in Behavioral Health or education equivalency for licensing • No experience required • Eligible remote residence in AL, FL, GA, IL, IN, IA, KS, KY, LA, MI, MO, NC, OK, SC, TN, TX, UT, WV, or WI • Required professional license/certification appropriate to the state of work location: Illinois LCPC, LCSW, or LMFT; Missouri LCSW, LMFT, or LPC; Oklahoma LADC, LBP, LCSW, LADC-MH, LMFT, LMSW, or LPC; Wisconsin LCSW, LMFT, LPC, MFT-TL, APSW, or LPC-IT • Experience with Epic • Crisis intervention skills • Strong communication skills • Teamwork skills • Ability to conduct assessments and safety planning via telephone or video • Ability to complete documentation and reporting in accordance with departmental, federal, and state requirements • Ability to work Monday through Friday, 8:00 a.m. to 4:30 p.m. • Ability to work in a constant state of alertness and safe manner • Physical ability to perform stated hearing, vision, lifting, standing, walking, keyboard, driving, and patient movement requirements
