📋 Description

• Serve as the primary point of contact for patients, responding to calls and providing timely, professional support throughout the intake process. • Help patients understand next steps, gather required information, and connect them with the appropriate care teams to support a positive patient experience. • Handle inbound and outbound communication with patients, providers, hospitals, physician offices, and healthcare facilities regarding referrals and care coordination. • Support timely patient onboarding by gathering, reviewing, and entering referral information accurately into company systems. • Receive and triage referral packets, medical orders, and supporting documentation to the appropriate teams for processing. • Coordinate home health services based on patient needs, geographic location, and insurance requirements. • Verify patient treatment timelines, including first-dose administration and home infusion scheduling needs. • Upload and maintain accurate patient records and documentation within the medical record system. • Partner with Intake Coordinators and nurses to ensure referrals move efficiently through the intake process. • Communicate referral updates and pending items to appropriate team members to help prevent delays in care. • Facilitate the accurate transfer of insurance information to Insurance Verification teams. • Deliver exceptional service to internal and external customers while maintaining patient confidentiality. • Support departmental operations through administrative tasks, phone support, and document distribution as needed. • Contribute to a collaborative, team-oriented environment focused on quality, accuracy, and patient-centered care.

🎯 Requirements

• High school diploma or GED. • Minimum 1 year of experience in a healthcare, medical office, pharmacy, home health, or related healthcare support environment. • Knowledge of medical terminology. • Strong verbal and written communication skills. • Ability to manage multiple priorities in a fast-paced environment while maintaining attention to detail. • Proficiency with Microsoft Office applications, including Outlook, Word, and Excel. • Strong organizational skills and commitment to providing excellent customer service. • Ability to maintain confidentiality and handle sensitive patient information appropriately.
