📋 Description

• Complete remote order entry, order verification, and drug verification • Perform transitions-of-care medication reconciliation • Navigate multiple pharmacy information systems • Provide drug information and therapeutic guidance to providers • Assess medication appropriateness, dosing including renal/hepatic adjustments, interactions, and compatibilities • Support specialized populations including pediatrics, geriatrics, pregnant/nursing patients, HIV patients, and patients with renal/hepatic impairment • Maintain knowledge of IV therapies, TPN, cardiac/narcotic drips, IV antibiotics, chemotherapy, and related therapies • Document interventions and clinical actions accurately and promptly • Communicate with physicians and nurses • Ensure compliance with HIPAA, pharmacy laws, and client policies • Participate in ongoing clinical, PIS, and policy training • Meet productivity and quality standards • Support multiple hospitals across several states through telepharmacy services

🎯 Requirements

• Bachelor's in Pharmacy required; PharmD preferred • At least 1 year of hospital pharmacy experience strongly preferred • Active and in good standing Illinois or California pharmacist license required • Willingness to obtain additional state licenses (Iowa, Nebraska, Hawaii) • Strong clinical knowledge and comfort working across multiple systems • Ability to work evenings, overnights, weekends, and holidays • Internet connection through cable broadband or fiber optic service with at least 10 Mbps download and 5 Mbps upload for home work • Ability to work across VPNs, automation systems, and multiple pharmacy information systems • Proficiency with Google Suite/Microsoft Office and Snagit • Ability to maintain required licenses, credentials, and system access
