📋 Description

• Complete remote order entry, order verification, and drug verification • Perform transitions-of-care medication reconciliation • Navigate multiple pharmacy information systems • Provide drug information and therapeutic guidance to providers • Assess medication appropriateness, dosing, interactions, and compatibilities • Support specialized patient populations, including pediatrics, geriatrics, pregnant/nursing patients, HIV, and renal/hepatic impairment • Maintain knowledge of IV therapies, TPN, cardiac/narcotic drips, IV antibiotics, chemotherapy, and related therapies • Document interventions and clinical actions accurately and promptly • Communicate with physicians and nurses • Ensure compliance with HIPAA, pharmacy laws, and client policies • Participate in clinical, PIS, and policy-update training • Meet productivity and quality standards

🎯 Requirements

• Bachelors in Pharmacy required; PharmD preferred • At least 1 year of hospital pharmacy experience strongly preferred • Active and in good standing IL or CA state pharmacist license required • Willingness to obtain additional state licenses (IA, NE, HI) • Strong clinical knowledge and comfort working across multiple systems • Ability to work evenings, overnights, weekends, and holidays • Cable broadband or fiber optic internet service with at least 10 Mbps download/5 Mbps upload for home working • Ability to work across VPNs, automation systems, and multiple pharmacy information systems • Familiarity with Google Suite/Microsoft Office and Snagit • Maintain required licenses, credentials, and system access
