📋 Description

• Conduct comprehensive virtual patient assessments, including medical and social history, visual examinations, limited physical examinations, and diagnostic evaluations • Use clinical decision-making grounded in disease pathophysiology and pharmacology • Manage acute conditions of post-acute patients, including stabilization of new admissions • Diagnose and treat acute and chronic medical conditions • Prescribe medications and develop treatment plans • Collaborate with healthcare professionals to ensure continuity of care • Use telehealth technology to deliver high-quality care • Maintain accurate and up-to-date patient records • Follow clinical guidelines, protocols, and regulatory requirements • Participate in educational programs that expand clinical knowledge and skills • Demonstrate accountability for professional practices

🎯 Requirements

• Master’s or Doctoral degree in Nursing from an accredited program • Current Nurse Practitioner (NP) license free of disciplinary action • Approved specialties include FNP, ACNP, ANP, and AGNP • NP licensure in listed primary or secondary license states: IL, MA, PA, CT, RI, MI, ME, NH, VT, IN, OH, WI, DE, MN, TX, FL, CO, MD, VA • Current AANP or ANCC Certification • Current CRNP License • Current NCCPA Certification • Current BLS Certification • Minimum of 5 years of clinical experience as a Nurse Practitioner • Minimum of 3 years of advanced practice provider experience within post-acute care/skilled nursing care preferred • Experience in telehealth or remote patient care preferred • Excellent communication and interpersonal skills • Strong attention to detail • Proficiency with telehealth technology and electronic health records (EHR) systems • High level of general computer proficiency and proficiency with Microsoft Office • Ability to work independently and as part of a multidisciplinary team • Compassionate and patient-centered approach to care • Availability for occasional weekend and/or evening work • Ability to meet productivity standards and perform essential job functions consistently and successfully under ADA, FMLA, and other applicable standards • HIPAA-compliant dedicated home-office space with privacy for PHI/PII and a professional video-conferencing backdrop • Access to Wi-Fi
