📋 Description

• Provide professional-level care coordination services within a managed care environment • Independently conduct comprehensive assessments of member needs using organizational platforms and available data sources • Provide short-term care coordination services aligned with member needs, benefits, services, and community resources • Develop, implement, and monitor individualized care plans with members and support teams • Conduct record reviews to identify care gaps and support care planning and service coordination • Coordinate services and resources across providers, agencies, and community organizations • Facilitate member transitions to ensure continuity of care • Exercise judgment in aligning member needs with appropriate resources

🎯 Requirements

• High school diploma or GED and six (6) years of experience working with mental health, substance use, and/or intellectual/developmental disability populations; OR Associate's degree and four (4) years of experience working with these populations; OR Bachelor's degree and two (2) years of experience working with these populations; OR equivalent combination of education and experience • Valid driver's license • Must reside within North Carolina • Must be able to travel within catchment as required • Preferred: Degree in Human Services • Preferred: Bachelor's degree • Resume must include employer name, dates of service, average hours worked per week, and essential job duties • Resume must include education details, including degree type, date awarded, institution, and field of study • Resume must include licensure/certification information, if applicable • Must pass a drug test as a condition of employment
