• Provide complex care coordination and monitoring to members with a primary intellectual or developmental disability and possible secondary behavioral-health or complex medical/physical-health needs. • Use person-centered planning methods to gather information, understand members' needs, and address individual requirements. • Provide education and support regarding members' rights and responsibilities. • Complete discovery activities, information gathering, and assessments as part of the planning process. • Ensure assessments and plans are updated when needed and when members' circumstances change. • Ensure quality support and adherence to waiver requirements. • Assist with integrated care management through multidisciplinary collaboration. • Travel within Trillium's catchment as required.
• Must meet QP status. • Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing with four years of full-time MH/DD/SA population experience; OR a master's degree in a human service field with one year of full-time MH/DD/SA experience; OR a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR a bachelor's degree in another field with four years of full-time MH/DD/SA experience. • Substance abuse professional alternatives require the stated supervised alcoholism and drug abuse counseling experience. • For members with LTSS needs, at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience is required; this may be concurrent with QP experience. • License and/or certification may be accepted as noted above. • Must have a valid driver's license. • Must reside within Trillium's North Carolina catchment; eligible counties are Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, and Wilson. • Must be able to travel within the catchment as required. • All candidates must pass a drug test as a condition of employment. • Resume must include employer names, dates of service, average weekly hours, essential duties, education details, and applicable licensure/certification information.
