📋 Description

• Provide behavioral health clinical coordination for members with complex behavioral, social, and psychosocial needs across population health programs • Assess member needs, risks, and barriers related to behavioral health and psychosocial factors • Coordinate services across providers, care teams, and community resources • Apply clinical judgment to support engagement in appropriate behavioral health and population health services • Monitor member progress and adjust coordination efforts to support improved outcomes • Facilitate communication among multidisciplinary teams to ensure coordinated care delivery

🎯 Requirements

• Master's degree in health and human services or related field • One (1) year of experience in a clinical setting working with the population served • Active North Carolina licensure as a Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Psychological Associate (LPA), Licensed Clinical Addictions Specialist (LCAS), or Registered Nurse (RN) • Valid driver's license • Must reside within North Carolina • Must be able to travel within catchment as required • Must pass a drug test as a condition of employment • Preferred: five (5) years post-Master's experience in a clinical setting with the BH/MH/SU/IDD population • Preferred: minimum of two (2) years Maternal health and Opioid experience
