• Provide direct supervision and oversight for an assigned team supporting members with intellectual/developmental/TBI disabilities. • Ensure staff use Trillium Connections workflows to complete comprehensive assessments and care plans as member needs change. • Establish a team-based, person-centered approach to coordinating members’ physical health, behavioral health, and social determinant of health needs. • Monitor teams to help ensure integrated care through multidisciplinary teams including members, guardians, primary care providers, behavioral health and IDD providers, specialists, nutritionists, and pharmacy. • Apply evidence-based and industry-standard best practices to develop processes, procedures, and workflows for effective care coordination and management.
• Bachelor’s degree or licensure as an RN, and five (5) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI; OR Master’s degree in a human services field or licensure as an RN, and three (3) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI. • Must have a valid driver’s license. • Must reside in North Carolina and within 75 miles Trillium’s South Central Region, which includes Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne counties. • Must be able to travel within catchment as required. • Work experience focused on working with Innovations Waiver preferred. • A minimum of two (2) years of experience supervising a team preferred. • All candidates must pass a drug test as a condition of employment.
