📋 Description

• Provide consultation-based behavioral health services within an integrated virtual primary care team • Conduct same-day consultation visits, scheduled video consultations, and focused interventions for referred patients • Provide behavioral intervention recommendations to referring primary care providers • Conduct risk assessments for crisis situations • Deliver evidence-based interventions for mental health symptoms, behavioral health issues, chronic disease management, and general health promotion • Practice collaborative, stepped, and measurement-based care with appropriate follow-up • Collaborate on clinical pathways and psychoeducational classes for priority populations • Develop patient education materials for common behavioral health conditions and problems • Provide collaborative decision support and interdisciplinary consultation to primary care teams • Educate primary care staff on brief behavioral and cognitive behavioral interventions • Support medication adherence and efficacy • Assist with triage, care coordination, and access to outside resources • Participate in clinical training, quality improvement activities, and staff meetings • Maintain clinical records and required administrative paperwork • Maintain virtual visibility and availability to the primary care team during clinic hours • Provide curbside provider-to-provider consultation through chat or phone • Deliver all patient care via HIPAA-compliant video telehealth

🎯 Requirements

• Master’s degree in Social Work from a CSWE accredited social work program or Master’s degree in Counseling from a CACREP or CORE accredited counseling program required • Licensed as an LCSW or LPC in the Commonwealth of Virginia, or active out-of-state license with Virginia licensure obtained within 18 months of hire • BLS Certification within 90 days of hire • Experience providing clinical interventions for a broad range of behavioral health conditions • Exposure to medically-related issues • Experience with pediatric populations and families for pediatric practices • Interest in a fast-paced primary care setting and interdisciplinary teamwork • Experience using a personal computer and computerized scheduling • Willingness to maintain flexibility in the clinic schedule • Effective interpersonal skills with patients and primary care team • Learning and growth mindset toward new topics and challenges • Passion for working in primary care and as part of a team • Must reside in Virginia and deliver all care from within the Commonwealth • Reliable high-speed internet required for video-based patient care • Private, confidential workspace suitable for clinical telehealth • Schedule includes 32 patient-facing hours and 8 administrative hours per week • Weekend and/or evening coverage required as part of the rotation
