📋 Description

• Provide consultation-based behavioral health services to patients in collaboration with primary care providers • Operate in a consultative role within an integrated behavioral health primary care model • Provide behavioral intervention recommendations to referring primary care providers • Conduct focused interventions through same-day virtual warm handoffs and scheduled video consultations • Provide focused assessments and collaborative care and treatment in primary care • Conduct risk assessments for crisis situations • Deliver evidence-based interventions for mental health symptoms, behavioral health issues, chronic disease management, and general health promotion • Practice collaborative, stepped, and measurement-based care with appropriate follow-up • Develop clinical pathways and psychoeducational classes with primary care team members • Contribute patient education materials for common behavioral health conditions • Provide collaborative decision support and interdisciplinary consultation • Educate primary care team members on brief behavioral and cognitive behavioral interventions • Support medication adherence and efficacy • Assist with triage, care coordination, and access to outside resources • Participate in clinical training, quality improvement activities, and staff meetings • Maintain clinical records and required paperwork • Maintain virtual visibility and availability to the primary care team during clinic hours • Provide curbside consultation via chat or phone • Deliver all patient care via HIPAA-compliant video telehealth

🎯 Requirements

• Must reside in Virginia and deliver all care from within the Commonwealth • Master’s degree in Social Work from a CSWE accredited social work program or Master’s degree in Counseling from a CACREP or CORE accredited counseling program required • Licensed to practice as an LCSW or LPC in the Commonwealth of Virginia, or obtain Virginia licensure within 18 months if actively licensed in another state • BLS Certification within 90 days of hire • Experience providing clinical interventions for a broad range of behavioral health conditions • Exposure to medically-related issues • Experience with pediatric populations and families for pediatric practices • Interest in working in a fast-paced primary care setting and interdisciplinary team • Experience using a personal computer and computerized scheduling • Reliable high-speed internet required • Private, confidential workspace suitable for clinical telehealth • Flexibility to support weekend and/or evening coverage in the seven-day clinic rotation • Effective interpersonal skills with patients and primary care team • Willingness to maintain flexibility in the clinic schedule • Ability to take a learning and growth mindset toward new topics and challenges • Passion for working in primary care and as part of a team
