📋 Description

• Coordinate and manage healthcare services for comprehensive, patient-centered care • Assess patients’ medical needs and develop individualized care plans • Advocate for patients’ rights, preferences, and access to appropriate services • Monitor patient progress, care-plan adherence, and intervention effectiveness • Adjust interventions as necessary • Document assessments, care plans, and outcomes accurately and promptly • Generate reports on patient status, care outcomes, and resource utilization • Educate patients on medications, treatment plans, and self-care practices • Provide emotional support and counseling to patients and families • Promote preventive care and wellness through health education and lifestyle guidance • Ensure compliance with URAC standards, CMSA Standards of Practice, applicable regulations, and Valenz policies • Collaborate with internal teams to identify health plan coverage savings opportunities • Perform other duties as assigned • Work remotely using company-provided equipment while maintaining a secure workspace and compliant technology environment

🎯 Requirements

• Active, unrestricted Registered Nurse (RN) license in state of residence • Compact License (NLC) accepted if residing in a compact state • 3+ years of nursing experience in an acute care setting • Ability to work in a deadline-driven environment • Strong attention to detail and organizational skills • Critical thinking and sound judgment in complex situations • Ability to escalate appropriately as needed • Excellent communication and interpersonal skills • Collaborative approach to care coordination • Reliable internet connection • Quiet workspace free from distractions • Adherence to company security protocols, including VPNs, secure passwords, and company-approved devices/software • Must be US based and located where work can be performed effectively and in compliance with company policies such as HIPAA • Availability to work until at least 4:00 PM Pacific Time • Case management experience is a plus • Bilingual Spanish is a plus • Relevant certification such as Certified Case Manager (CCM) or Chronic Care Professional (CCP) is a plus • Willingness to obtain CCM within 18 months of hire is a plus
