• Report to the Chief Medical Officer and provide leadership for Valley Healthcare System's organization-wide quality and performance improvement program • Develop and revise the annual Quality Improvement program, evaluation, and work plan • Integrate quality planning into organizational strategic and operating plans • Develop a data-driven culture supporting compliance and continuous quality improvement • Lead complex improvement initiatives and resolve strategically important quality issues • Design, plan, and implement performance improvement systems • Review processes, policies, and procedures; identify best practices and recommend changes • Conduct quality audits related to Uniform Data System reporting, Patient-Centered Medical Home standards, HEDIS measures, Meaningful Use requirements, and other applicable programs • Collect, prepare, trend, and analyze aggregate performance improvement and quality monitoring data • Present data and findings to executive leadership, committees, and departments • Review clinical documentation for completeness, accuracy, quality, and compliance • Identify audit outliers and quality concerns; develop and monitor corrective action plans • Research clinical standards, benchmarks, and evidence-based criteria • Assist with provider documentation and quality audits at least twice annually • Coordinate care-gap closure and payer quality initiatives for Medicaid and managed care patients • Distribute and track patient appointment agendas and quality documentation from contracted partners • Promote healthcare integration and Patient-Centered Medical Home principles • Coach and support staff in completing quality improvement deliverables • Provide consultation, technical assistance, and training on quality tools, methods, and measures • Coordinate and provide functional direction to Data Analysis, Medical Records, and Risk Management personnel • Consult with leadership, managers, providers, and staff on quality, compliance, risk, and performance matters • Monitor regulatory requirements, professional standards, accreditation expectations, and industry practices • Support compliance with federal, state, and local requirements, ethical standards, confidentiality guidelines, and HIPAA policies • Identify methods to reduce organizational, clinical, and operational risk • Serve on and lead meetings for the Quality Improvement Committee and Risk Management and Compliance Committee • Develop strategic partnerships and seek funding to strengthen quality management programs • Maintain quality management knowledge through continuing education and professional development • Lead or assist with special projects assigned by the Chief Executive Officer or Chief Medical Officer
• Bachelor's degree in social work, healthcare administration, business administration, or a closely related field from an accredited institution • Master's degree preferred • Seven to ten years of progressively responsible managerial experience in a healthcare, social service, or business environment • At least five years of experience developing, implementing, and maintaining quality management systems encompassing quality assurance, performance improvement, and compliance activities • Experience with NCQA accreditation requirements and Medicaid quality programs preferred • Demonstrated experience in performance improvement, statistical analysis, information systems, data visualization, and outcome measurement strongly preferred • Working knowledge of continuous quality improvement, quality assurance, risk management, healthcare compliance, clinical documentation auditing, and corrective action planning • Knowledge of HIPAA, NCQA, PCMH, UDS, HEDIS, Medicaid managed care quality requirements, and other applicable healthcare standards • Ability to analyze complex quantitative and qualitative data, identify meaningful trends, and communicate findings through accurate reports, graphs, and presentations • Strong project management, facilitation, training, coaching, problem-solving, and organizational skills • Ability to collaborate across clinical, administrative, and executive teams while exercising sound independent judgment • Strong written and verbal communication skills, attention to detail, discretion, and commitment to patient privacy • Proficiency with electronic health records, spreadsheets, databases, presentation software, and other quality measurement and reporting tools • Ability to comply with company policies and Federal, State, and Local medical service requirements • Required to sign a Pledge of Confidentiality and comply with HIPAA Privacy & Security requirements • Requires prolonged sitting, standing, bending, stooping, stretching, eye-hand coordination, and manual dexterity • Requires corrected vision and hearing to normal range • Frequent exposure to communicable diseases, toxic substances, and other clinic-environment conditions • Occasional evening or weekend work
