📋 Description
• Provide proactive intervention and coordination of care to eligible Vaya Health members. • Conduct assessments and identify needs for members. • Collaborate with care teams and community stakeholders to enhance care processes. • Ensure compliance with health regulations and standards.
🎯 Requirements
• Master's degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. • 2 years of experience working directly with individuals with BH conditions. • 2 years of prior Long-term Services and Supports and/or Home Community Based Services coordination, care delivery monitoring and care management experience. • Valid licensure required.
