📋 Description

• Conduct a high volume of outbound patient calls, ramping from 5–20 patient connections daily in the first month to 22+ by month three • Build trust, rapport, and accountability with assigned patients using Motivational Interviewing and clinical best practices • Assess patient risk stratification and prioritize outreach across the patient panel • Support low-risk patients in maintaining stability and help higher-risk patients improve • Reduce avoidable emergency room visits and hospital stays • Collaborate with patients to develop SMART goals and patient-centered care plans • Support PCP visits, treatment adherence, care navigation, and routine assessments • Translate patient conversations into clear actions and accurate clinical documentation • Coordinate with the broader care team to support patient progress and improved health outcomes • Generate patient success stories through effective patient engagement

🎯 Requirements

• Experience in one or more of the following roles: RN, LVN, LPN, MA, MSW, LCSW, Medical Case Manager, Care Manager, Health Coach, Community Health Worker, Wellness Coach, Nutritionist, or Health Outreach Specialist • Demonstrated clinical experience working with patients with one or more chronic conditions • Experience developing patient-centered care plans and SMART goals supporting patient health outcome goals • Bilingual or multilingual communication skills required • Behavioral Health/Mental Health Integration experience highly preferred • Motivational Interviewing techniques preferred, plus ability to quickly learn structured clinical conversation frameworks • Prior use of care management platforms or EHRs • Prior use of MS Office Suite, especially Teams and Excel • Knowledge of HIPAA and patient privacy requirements • Ability to manage a full patient panel, prioritize outreach, and maintain accurate documentation • Ability to work remotely from Jacksonville, Florida, statewide
