📋 Description

• Serve as the client’s primary point of contact under the direction of the ECM Clinical Manager • Coordinate with doctors, specialists, pharmacists, social services providers, caregivers, family support persons, and other Care Team members • Manage client cases and coordinate health care benefits • Provide education and facilitate timely, cost-effective access to care • Promote wellness, recovery, independence, resilience, and member empowerment • Assess member needs across physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community services • Oversee development of client care plans and goal setting • Offer office-based, telehealth, or field-based services where members reside, seek care, or can access services most easily • Connect clients to social services and supports • Advocate for clients with health care professionals • Use evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care • Conduct outreach and engagement activities and log activity in the CRM system • Evaluate client progress and update SMART goals • Provide mental health promotion • Arrange transportation such as ACCESS • Complete documentation and outcome measures within care-plan timeframes • Maintain current patient health records in the EMR and other business systems • Complete monthly reporting for program compliance • Attend assigned training

🎯 Requirements

• Applicants must be located in Arizona, California, Colorado, Illinois, Nevada, Oregon, Texas, Utah, or Washington • Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time, with the possibility of evenings and weekends • Active, unrestricted LCSW, LMFT, or LPCC license in California required • 1+ year of experience as a care manager, care navigator, or community health worker supporting vulnerable populations • Working knowledge of government and community resources related to social determinants of health • Excellent oral and written communication skills • Positive interpersonal skills required • General computer skills • Working knowledge of Google Workspace, MS Office, and the internet • Bilingual English/Spanish highly preferred • Background check, potentially including a drug test or other health screenings, required prior to employment • Employees in patient, client, or customer-facing roles must be vaccinated against influenza • Identity and work eligibility verification using E-Verify upon hire • Must be legally authorized to work in the United States
