📋 Description

• Conduct telephonic assessments to identify members’ medical, behavioral, and social needs • Develop and coordinate personalized care plans to support overall wellness • Collaborate with healthcare providers and multidisciplinary teams to remove barriers to care • Utilize clinical judgment to address complex health conditions and improve outcomes • Support members through transitions of care and connect them with appropriate resources • Maintain compliance with regulatory and company case management standards • Work Monday-Friday, 9:00 AM-5:30 PM EST, with occasional evenings, weekends, holidays, and limited travel up to 10% • Work remotely from a designated home workspace within North Carolina • Navigate multiple systems and applications to manage case work efficiently

🎯 Requirements

• Active, unrestricted RN license in North Carolina • Must reside in North Carolina • Minimum 2 years of RN clinical experience • Associate Degree in Nursing • Experience in any clinical setting considered, including ICU, NICU, Emergency Department, Med/Surg, Telemetry, OR, Home Health, Hospice, Long-Term Care, Infusion, and Orthopedics • Strong communication and member engagement skills • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • Quiet, secure, private designated home virtual work location compliant with CVS Health and HIPAA guidelines • Ability to provide workspace furniture, such as a desk or standing desk • Comfortable setting up and using multiple monitors and navigating multiple applications simultaneously • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace applications • Ability to log in to secure systems such as VPN and EHR portals • Ability to follow computer security practices, manage strong passwords, and recognize suspicious emails or links • Ability to resolve common technical issues independently and contact IT for unresolved issues • Openness to learning new skills as the workplace evolves • Preferred: Certified Case Manager (CCM) • Preferred: BSN • Preferred: Case Management or Managed Care experience • Preferred: Discharge Planning experience • Preferred: Proficiency with Microsoft Office Suite
