• Operate within pharmacist-led clinical programs focused on chronic disease, quality improvement, and medication optimization • Outreach, enroll, and engage eligible patients through education, follow-up, and telephonic medication support • Conduct Medication Reconciliation, Medication Therapy Management, and Comprehensive Medication Reviews • Provide medication-use, adherence, self-management, and escalation-of-care education • Make actionable medication recommendations and collaborate with providers to support medication management and practice adoption • Support clinical programs through pharmacy-related consultation • Drive medication adherence and address quality gaps • Collaborate with interdisciplinary teams to develop patient-specific clinical action plans, reconcile medications, and adjust medications • Drive provider uptake of drug-related clinical decision-support recommendations • Provide feedback to improve decision-support logic, usability, and impact • Collaborate with medical officers, directors, practice transformation managers, care teams, and practices • Support consistent clinical standards, workflows, and messaging across markets • Help develop specialty-medication, high-cost-drug, and pharmacy-optimization initiatives • Partner with pharmacy and medical leadership on value-based drug strategy • Document clinical activity in compliance with state and federal regulations and HIPAA requirements • Serve as a pharmacy information resource for providers and internal clinical teams • Remain current on clinical guidelines, quality measures, and pharmacy best practices
• Doctor of Pharmacy (PharmD) • Active pharmacist license in good standing in one of: CA, WA, KS, OH, NY, NJ, FL, MD, NH, MS, LA • Experience in Medication Therapy Management and pharmacist-led clinical programs • Strong understanding of evidence-based guidelines and quality measures • Experience or interest in value-based care, population health, or ACO environments • Strong clinical judgment and ability to make independent recommendations • Ability to work across clinical programs, data-driven tools, and operational teams • Comfort working with partial or evolving clinical information • Ability to drive results, not just provide clinical input • Excellent communication skills with providers, patients, and internal stakeholders • Organized, adaptable, and effective in fast-paced, evolving environments • High degree of professionalism, integrity, and accountability • Ability to work in the United States; application asks whether sponsorship will be required • UMPJE preferred
