📋 Description

• Ensure accurate and timely documentation of all TOC-related activities in WelTeam and Athena. • Create authorization shells for TOC-related services (e.g., transportation, SNF stays). • Coordinate transportation for participant discharges and transfers to and from skilled nursing facilities (SNFs). • Arrange delivery of durable medical equipment (DME) and medications upon discharge. • Monitor designated QuickCap and Athena inboxes and respond appropriately. • Collaborate closely with the TOC Nurse to support care coordination needs. • Maintain prompt, clear communication with WelbeHealth Clinic staff to provide TOC updates (including new encounters such as admissions or ER visits) and obtain information necessary to complete assigned tasks. • Communicate directly with hospital staff as part of coordinating participant care needs. • Participate in TOC huddles and document relevant updates in Athena. • Other duties as assigned.

🎯 Requirements

• High school diploma or GED required. • Medical Assistant certification preferred. • Experience in healthcare, care coordination, or administrative support preferred. • Experience with Transitions of Care and related tasks preferred.
