📋 Description

• Manage the intake process for new patients, including receiving referrals, insurance verification, and scheduling initial assessments • Serve as the primary point of contact for patients, families, healthcare professionals, and referral sources • Ensure accurate and timely documentation of patient information, medical histories, assessments, care plans, and communications • Collaborate with interdisciplinary team members to develop individualized care plans • Coordinate medical equipment, supplies, therapy services, and community resources • Educate patients and families about conditions, treatment options, self-care techniques, and support services • Monitor patient progress and treatment response, reassessing needs and adjusting care plans • Coordinate transitions between care settings, including hospital discharge to Home Health and/or Hospice services • Participate in quality assurance activities, audits, reviews, and performance improvement initiatives • Collaborate with physicians, nurses, therapists, and social workers to ensure coordinated care

🎯 Requirements

• Associates Degree or Certification in Nursing • Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC) • Bachelor’s degree in nursing preferred • Two (2) years of Home Health and/or Hospice experience preferred • Two (2) years of experience in utilization review/chart audit preferred • Scheduled weekly hours: 40 • Non-exempt position
