📋 Description

• Triage incoming patient referrals and assess patient needs • Provide appropriate clinical dispositions and facilitate referrals to primary and specialty care providers • Educate patients on immediate care advice, preventive behaviors, diagnoses, treatments, side effects, and post-treatment care • Accompany patients to appointments and coordinate travel, accommodations, and itineraries • Coordinate multidisciplinary care across the care continuum • Collaborate with care teams to expedite patient workups, treatment, survivorship, and end-of-life care • Schedule and coordinate clinic appointments, e-visits, and telehealth visits • Communicate with patients, providers, clinicians, referring providers, and primary care providers • Assess and document patient needs, goals, preferences, health literacy, and educational needs • Bridge gaps in care and troubleshoot care transitions and barriers • Document all patient conversations in the medical record • Provide direction and guidance to Patient Navigators • Participate in service line team meetings

🎯 Requirements

• Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse, OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) • Three years of registered nursing experience • Ability to handle and maintain confidential information • Ability to work in a fast-paced and rapidly changing environment • Excellent oral and written communication skills • Strong organizational and motivational skills • Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping • Ability to sit for extended periods of time • Availability for 7a–7p shifts with weekend and night rotations
