• Provide medical and patient support to patients, caregivers, and health care providers • Educate patients and providers on diseases, therapies, and Patient Support services using approved materials • Conduct patient assessments and answer program, disease, and product-related inquiries • Act as a liaison between patients, families, health care providers, pharmacies, and clinics regarding medication access and treatment logistics • Provide ongoing adherence support and refer clinical questions beyond program materials to healthcare providers • Document patient information and interactions in electronic patient records • Review and update patient files throughout the patient journey • Follow case-management protocols, including data collection, clinical monitoring, follow-up, therapy disruption/discontinuation monitoring, compliance, and adverse-event reporting • Collaborate with reimbursement specialists and physicians to secure therapy funding and identify alternative funding sources • Coordinate and track patient services according to KPIs to expedite therapy access • Participate in specialized therapy training, pharmaceutical meetings, patient meetings, conferences, and patient advocacy activities • Maintain communication with the manager, team, and client regarding patient case management • Assist with training new Nurse Case Managers and provide feedback on protocols, databases, processes, and inter-departmental relationships • Support development and implementation of disease-management programs, including clinical requirements, patient materials, compliance parameters, needs assessments, and protocols • Report adverse events and severe adverse events according to program procedures • Perform other duties as assigned and work on standby/on call when required
• Baccalauréat ou diplôme d’infirmier(ère) autorisé(e) (IA), diplôme d’études supérieures ou expérience spécifique de gestion d’un cabinet est préférable • Five (5) years of nursing experience • Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required • Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance or related fields • Experience in healthcare and case management in medical/hospital settings, insurance or bio-pharmaceutical industry preferred • Fluency in French and/or English may be required depending on program and geographic location • Experience in hematology and/or nephrology is an asset • Strong patient-centered focus • Ability to multitask and work under pressure • Excellent customer service, telephone, communication, problem-solving, teamwork, negotiation, analytical, mathematical, organizational, interpersonal, leadership, and presentation skills • Ability to communicate and understand medical information • Proficiency in internet research and computer skills • Proficiency in Microsoft Word, Excel, PowerPoint, and other Office programs • Ability to work independently and consistently meet deadlines • Willing to travel at least 15% of the time each year • Ability to work on standby/on call as required
