📋 Description

• Support patients and care teams across Standard and Complex Care models • Supervise Med-Psych and Psych APCs and their care pods • Provide virtual mental and physical health care via video, phone, and text • Lead integrated behavioral health services for patients with mild to moderate mental health and substance use needs • Act as the psychiatric consultant, collaborating with primary care and APCs • Provide direct and consultative care for patients with serious mental illness and/or severe substance use disorders • Ensure multidisciplinary care plans that reduce avoidable utilization and increase patients’ healthy days at home

🎯 Requirements

• MD or DO in Psychiatry with dual Board Certification in Psychiatry and Internal Medicine / Family Medicine • Fellowship in consult liaison Psychiatry a plus • Fellowship/additional training or experience in addiction medicine a plus • Comfort and experience in managing both psychiatric and medical needs for patients • Active Medical License in state of employment and willingness to obtain additional licensure as requested • Experience building and leading multi-disciplinary teams, supervising clinical team members, and have held leadership and/or administrative roles in clinical organizations or other innovative care models • Experience with change management and process improvement theory, techniques and practical application • Experience using a metrics-driven approach to analyze cost, quality, and satisfaction data to drive clinical strategy and program redesign • Comfort with working in an electronic medical record • Valid unrestricted driver's license and access to an insured vehicle for daily use • 3+ years in an outpatient mental health practice and/or primary care setting preferred • Experience / additional training in Collaborative Care models preferred • Experience supervising Advanced Practice Clinicians preferred • Experience working in value-based care setting and/or managed care preferred
