• Review provider SOAP notes, medical records, patient activity, medications, hospitalizations, referrals, laboratory results, and outstanding orders • Prepare concise patient-specific chart summaries and identify discrepancies or unresolved care needs requiring clinical review • Create and maintain electronic care plans under the direction of the clinical team • Conduct structured monthly patient or caregiver outreach using approved workflows • Review symptoms, function, medication adherence, patient goals, and barriers to care • Reinforce provider-approved treatment plans and provide approved educational information • Coordinate appointments, referrals, records, transportation, equipment, pharmacy needs, community resources, and outside-provider communication • Follow up on hospitalizations, emergency visits, rehabilitation stays, specialist recommendations, tests, imaging, and outstanding orders • Recognize and escalate urgent symptoms, abnormal findings, medication concerns, and safety issues to licensed clinical staff • Document outreach, care coordination, education, care-plan updates, escalations, and follow-up tasks in the electronic health record • Track qualifying CCM time, work queues, monthly follow-up lists, and task completion • Protect patient information and comply with HIPAA, company, client, and payer requirements • Participate in training, quality reviews, audits, coaching, and corrective-action activities • Remain within the role’s non-clinical scope and do not diagnose, prescribe, interpret results independently, or provide unauthorized medical advice
• At least one year of experience in a medical office, primary care practice, care management program, healthcare call center, home health organization, or related healthcare setting • Experience using an electronic health record and reviewing patient charts • Familiarity with common chronic conditions, medical terminology, medications, referrals, laboratory reports, and healthcare documentation • Strong written and verbal English communication skills • Strong telephone communication and patient-engagement skills • Excellent attention to detail and documentation accuracy • Ability to conduct organized, patient-centered conversations using approved workflows • Ability to identify missing, overdue, inconsistent, or unresolved care needs • Ability to distinguish routine administrative needs from concerns requiring licensed clinical review • Strong organizational, time-management, and follow-up skills • Ability to manage multiple patients, work queues, deadlines, and monthly tasks • Ability to protect confidential patient information and follow established protocols • Comfortable working during U.S. business hours or graveyard shifts in the Philippines • Reliable internet connection and a private, secure remote working environment
