📋 Description

• Conduct online “Video Visits” with health plan members • Engage with health plan members primarily online, helping to close gaps in care • Provide 40 hours of availability weekly • Execute a strong visit completion rate • Maintain compliance with operational, clinical, and member experience key performance indicators

🎯 Requirements

• Full-time, board-certified nurse practitioners or physician assistants • Experience with Signify Health In-Home Health Evaluations, Video Visits, telehealth visit delivery • Maintain training, affiliation, and compliance with Video Visit requirements • Be trained and credentialed for all health plan clients in primary coverage state(s) • Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs) • Able to obtain and maintain multi-state licenses as required
