📋 Description

• Assess and evaluate member's needs and requirements to achieve and/or maintain optimal wellness state • Guide members/families toward and facilitate interaction with resources appropriate for care and wellbeing • Employ a variety of strategies, approaches and techniques to manage a member's health issues • Identify and resolve barriers that hinder effective care • Ensure patient is progressing towards desired outcomes by continuously monitoring patient care through assessments, data, conversations with member, and active care planning • Understand work area professional concepts/standards, regulations, strategies and operating standards • Make decisions regarding own work approach/priorities, and follow direction • Follow established guidelines/procedures • Provide care telephonically and ensure effective progress of the patients

🎯 Requirements

• Bachelor’s degree in a health or social service-related field or LPN licensure in Illinois • 2 or more years of relevant/ case management experience • Experience utilizing Microsoft Word, Excel and Outlook • Experience with electronic information applications/software programs • Exceptional verbal/written communication and interpersonal skills • Bilingual English/Spanish /Polish/Arabic (preferred) • Certified Case Manager (CCM) (preferred) • Master’s degree in a health or social service-related field (preferred) • Prior experience with Medicare & Medicaid recipients (preferred) • Previous experience with electronic case note documentation and experience with documenting in multiple computer applications/systems (preferred) • Experience with health promotion, coaching and wellness (preferred) • Knowledge of community health and social service agencies and additional community resources (preferred) • Experience with case management, discharge planning and patient education for adult acute care (preferred) • Preferred residing in IL (preferred)
