📋 Description

• As an APP with Imagine Pediatrics, you influence and ensure the best clinical care for your patients. • Manage a panel of complex pediatric patients – Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination. • Work in a multidisciplinary team – Work with pediatricians, nurses, pharmacist, dietitian and social workers to provide wrap-around care in a virtual-first model. • Empower families – Educate and support parents to navigate their child’s health journey with confidence, ensuring long term health success. • Deliver high-quality virtual care – Conduct virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, and even in-home virtual assessments. • Ensure continuity of care – Step in when needed to bridge care gaps, coordinating prescriptions, and streamline referrals, to remove barriers to care.

🎯 Requirements

• Minimum 5 years’ experience with a pediatric population required, Community Based Health or Complex Care strongly preferred. • You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification. • Active NJ, NC, FL, or NY licensure is required, and an additional license in GA or DC is preferred • Experience and comfort with children with complex medical needs • Experience with Medicaid and/or Community health preferred • Virtual care experience a plus. • Strong desire to improve the quality of life for pediatric patients and their caregivers. • Ability to lead and guide a clinical team. • Bilingual Spanish is strongly preferred.
