📋 Description

• Optimize and clarify the medical care needs and positive functional capacity profile of rehabilitation clients • Provide vocational counseling and job placement support for rehabilitation clients • Conduct profiling and job application training • Source job opportunities through individualized educational support • Document counseling and placement activities in writing • Serve as a person-centered navigator, systematically coordinating services and support • Coordinate case-related activities with funding agencies and service providers within the regional network • Collaborate with rehabilitation facilities, social benefit providers, counseling centers, and self-help groups

🎯 Requirements

• Completed degree in a humanities or social science discipline, such as social work, education, or psychology, plus at least two years of professional experience in vocational (re)orientation counseling • A completed psychology degree additionally requires proof of at least 40 teaching units in social welfare law • Alternatively, completed vocational training in the health and social care sector or further training in rehabilitation, in each case with at least five years of professional experience in vocational (re)orientation counseling • Proof of methodological and social welfare law expertise: 160 teaching units in counseling skills and 40 teaching units in social welfare law • Customer focus, strong motivational skills, empathy, and persuasiveness • Desirable: Case Management training or CDMP qualification • Knowledge of the structures of social benefit providers and rehabilitation law • Experience in vocational and medical rehabilitation, as well as solution-focused case management • Good knowledge of MS Office, especially Microsoft Word, and internet applications • Willingness to travel locally, preferably with your own car
