📋 Description

• Lead and oversee the clinical outreach program to manage complex patients • Lead and support a team of nurses coordinating care for patients • Build, create, and review patient care plans for complex patients • Oversee nurse outreach to complex patients • Handle clinical escalations and complex patient scenarios • Collaborate with nurses, care coordinators, specialists, pharmacists, and other healthcare professionals to optimize patient outcomes • Partner with the Chief Medical Officer and other clinical staff to develop and enhance clinical operations • Partner with the Director of Care Coordination to ensure effective onboarding and ongoing development of team members • Advocate for, support, and motivate members to manage their health and healthcare • Maintain positive working relationships with members, families, providers, and external partners • Work effectively in a multicultural setting serving diverse populations • Build trust, actively listen, communicate intentionally, collaborate, problem-solve, and influence positive outcomes • Sit for long periods of time

🎯 Requirements

• Active, unrestricted Nurse Practitioner (NP) license in Texas; ability to obtain additional state licenses if needed • Full and current board certification in an area of clinical specialty from ANCC, AANPCB, or another nationally recognized certifying body • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE • 5+ years of progressive leadership experience managing a team • 2+ years of remote work experience • Experience in Chronic Care Management • Dedicated home office for remote work • Experience working with Veterans and/or Medicaid and/or Dual Eligible patients • Strong interpersonal, verbal and written communication, and problem-solving skills • Experience working with customers over the phone and by text message • Experience documenting case notes in a care management or electronic health record platform • Experience with motivational interviewing • Additional licensure in Louisiana, Mississippi, Arkansas, Oklahoma, Missouri, Kansas, Nebraska, Utah, Wyoming, or Colorado preferred • Adult Nurse Practitioner, Family Nurse Practitioner, Psychiatric Mental Health Nurse Practitioner (PMHNP), or Clinical Nurse Specialist (CNS) provider types preferred • Experience with Apple computers (i.e., MacBook) preferred • Bilingual fluency in Spanish preferred
