Sprinter Health is a hybrid in-home and virtual primary care provider that expands access to proactive, preventive healthcare by combining community-based, W-2 “Sprinters” (trained phlebotomists with medical assistant and community health worker skills) who perform hands-on diagnostics and wellness checks in patients’ homes with a virtual clinical team of physicians, nurses, pharmacists, and care navigators. 📋 Description

• Conduct virtual adult and/or pediatric wellness visits • Perform health assessments and other virtual healthcare services • Review caseload and prepare technology and tools for patient service • Navigate the daily schedule using company technology • Partner with clinical in-home team members (Sprinters) visiting patients’ homes • Build welcoming connections with diverse patients • Collect relevant patient information and perform various services • Conduct health risk assessments, medication reviews, and cognitive screenings • Provide patients with educational information about health and well-being • Collaborate with Sprinters to evaluate vital signs and arrange blood draws • Carry out tasks addressing patients’ specific needs • Provide care coordination and management • Deliver preventive care interventions and medication management • Refer patients to specialists and community resources • Document findings

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in California • Credentialed Medicaid provider for the state of California (Medi-Cal) • Prior experience performing wellness visits • Professionalism, integrity, accountability, confidentiality, care, and compassion • Willingness to perform last-minute problem solving and out-of-the-box thinking • Technologically savvy and comfortable using laptops or mobile devices for charting • Comfortable using HIPAA-secure messaging apps for care coordination • Strong written and verbal communication skills • Ability to work independently or in a team environment • Must pass a national background check and valid clinical license search • Application asks about prior Medicare and/or Medicaid Annual Wellness Visit experience • Application asks about virtual experience as a Family Nurse Practitioner • Application asks about active Family Nurse Practitioner licenses in other states • Application asks about current prescriptive authority in licensed states
