📋 Description

• Serve as Patient Care Manager Regional Floater for AccentCare’s home health operations • Provide patient care management across regional home health needs • Apply accepted professional standards and practices, Medicare Conditions of Participation, and federal, state, and local regulatory requirements • Support exceptional care delivery in collaboration with the home health team

🎯 Requirements

• Previous experience in home care setting with two years management or supervisory experience, preferred. • Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements • Registered nurse with current licensure to practice nursing in the practicing state.
