📋 Description

• Conduct intake assessments to evaluate Member readiness and determine appropriate level of care • Facilitate group and one-on-one counseling sessions using Affect’s evidence-based curriculum • Provide crisis support and coordinate appropriate referrals when needed • Maintain timely, accurate documentation in the electronic medical record (EMR) system • Collaborate with clinical supervisors on individualized treatment and discharge planning • Ensure compliance with HIPAA and all relevant state regulations • Verify credentials through the State Board’s website as part of the application process

🎯 Requirements

• Currently licensed in Arizona with an active and verifiable LCSW, LPC, LMFT, LIAC, LACT, or LAAC license; credentials must be verified through the State Board’s website • If residing outside Arizona, also hold an active and verifiable license in the state of residence • Proficient with Microsoft Office or Google Suite, EMR systems, and telehealth platforms • Private, HIPAA-compliant workspace • Reliable internet • Comfortable working independently and collaboratively in a fast-paced, mission-driven startup environment • Passion for supporting individuals in recovery and building therapeutic relationships
