📋 Description

• Assess patients’ medical and mental health needs • Provide plans of care that proactively manage patient conditions and address barriers to care • Ensure compliance with local, state, and federal agencies related to clinical services • Prescribe medications • Order lab work, diagnostic procedures, and consultations • Monitor patients’ compliance and response to treatment • Modify treatment plans as needed • Collaborate with providers, case managers, facility partners, and health plan leadership • Implement Treat in Place Protocols • May provide training and advice to facility staff

🎯 Requirements

• APRN or PA license required • Degree from an accredited APRN or PA program • 3 years’ experience in clinical nursing or rehabilitation with geriatric populations and/or Fee for Service experience • Experience with Electronic Health Records • Working knowledge of Microsoft applications, including Word, Outlook, and Excel • Bachelor’s or master’s degree preferred • Ability to provide independent judgment in treating acute and chronic conditions • Compliance with federal and state regulations
