📋 Description

• Manage patient health needs as part of an interdisciplinary team through virtual primary care • Assess, diagnose, and create treatment and management plans appropriate for age, patient acuity, and clinical condition via virtual health modalities • Provide scheduled virtual visits • Perform dedicated asynchronous In-Basket cross-coverage for PCPs who are out • Participate in In-Basket work, workflows, and coverage for team members who are out • Safely triage and manage unfamiliar patients in a virtual setting • Navigate electronic health records and pivot between scheduled video visits and asynchronous chart review

🎯 Requirements

• Nurse Practitioner: Master’s or Doctorate’s degree from an accredited Nursing Program • Physician Assistant: Bachelor’s or Master’s degree from an accredited PA Program • Nurse Practitioner: Active RN and APNP license in the State of Wisconsin • Nurse Practitioner: Active board certification as an NP in Adult, Adult/Gerontology Primary Care, or Family • Physician Assistant: Active PA license in the State of Wisconsin • Physician Assistant: Active PA board certification • Active DEA license or ability to apply for such license prior to or upon hire • CPR certification and advanced life support certifications if relevant to the practice area (ACLS, PALS, NRP) • Proficiency navigating electronic health records • Strong independent clinical judgment • Ability to safely triage and manage unfamiliar patients in a virtual setting • Adaptability between scheduled video visits and asynchronous chart review • Work schedule: 100% FTE, Monday–Friday, between 8:00 AM–5:00 PM; no holidays or call
