📋 Description

• Coach, develop, supervise, direct, and evaluate Support Coordination professionals • Monitor staff performance, service performance, quality, productivity, compliance, and utilization benchmarks • Provide or coordinate staff training and developmental opportunities • Ensure effective, person-centered support coordination activities for ECF CHOICES members • Oversee development and implementation of person-centered support plans, including employment and integrated community-living outcomes and goals • Authorize services outlined in support plans and monitor progress toward goals • Provide real-time guidance, performance coaching, issue resolution, operational support, communication, and mentoring • Ensure adherence to ECF CHOICES guidelines, policies, procedures, TennCare, and BlueCare requirements • Periodically carry a member caseload when required by census fluctuations or staffing coverage • Maintain relationships with members, families, providers, and community partners • Support timely assessments, visit completion, critical-incident follow-up, and service coordination • Travel 90% of the day

🎯 Requirements

• Sponsorship is not available for this role • RN with an active license in Tennessee, or an active license in the employee’s state of residence if participating in the Nurse Licensure Compact Act; or Master’s in Social Work with an active unrestricted license; or BS degree in social work, nursing, education, or related field approved by TENNCARE; or meet federal requirements for a Qualified Intellectual or Developmental Disabilities Professional (QDDP/QIDP) • 3 years of supervisory experience • 2 years of experience supporting people with intellectual and developmental disabilities • Proficient in Microsoft Office (Outlook, Word, Excel, and PowerPoint) • Proven analytical and problem-solving skills, including ability to perform non-routine analytical tasks • Proven leadership abilities that promote employee growth and development • Excellent oral and written communication skills • Strong interpersonal and organizational skills • Valid driver’s license • Must comply with BCBST Driver Safety Program requirements when routinely operating a company, personal, or rental vehicle, including auto insurance and acceptable motor vehicle record • Various immunizations and/or associated medical tests may be required
