📋 Description

• Lead RN and LPN teams responsible for alert review and triage across the chronic care population. • Coach and develop clinical staff, setting performance standards aligned with guideline-directed care for hypertension, diabetes, and heart failure. • Own staffing, scheduling, and workload distribution to efficiently cover alert volume. • Build and refine triage and alert workflows so concerning vitals reach the right clinician quickly. • Partner with cross-functional teams to connect surfaced risk with clinical intervention between visits. • Analyze team performance data to identify bottlenecks, strengthen escalation pathways, and scale operations as patient volume grows. • Report to clinical operations leadership and partner with providers and care teams.

🎯 Requirements

• Active, unencumbered compact RN license required. • Minimum of 5 years of clinical experience as a Registered Nurse, including work in triage, remote patient monitoring, or chronic disease management for conditions such as hypertension, heart failure, and diabetes. • Minimum of 3 years of nursing leadership experience with full supervisory responsibility for clinical teams, including hiring, performance management, coaching, and development in high-volume, queue-based, or remote care delivery environments. • Track record of scaling clinical operations in a growth-stage or early-stage company. • Proficiency with EHR systems and performance data tools, with the ability to work across platforms such as Athena, Epic, Zendesk, Notion, and G Suite. • Fluency with AI-assisted clinical tools such as Gemini, Open Evidence, or comparable platforms. • Ability to speak credibly with providers and patients about where AI supports care delivery and where human judgment remains non-negotiable. • Position is based in the United States.
