📋 Description

• Receive new patient referrals from clients • Create charts for all new patients • Input medical information in new chart • Help Care Coordinators with patient needs as applicable • Foster relationships with their team to ensure the CareHarmony culture is positive and supportive • Perform other duties as applies • Assist the team in referral management for High-Risk patients and help with patient needs under the direction of the department manager

• Great attention to detail • Excellent organizational skills • Excellent verbal and communication skills • Excellent professional presence when dealing with colleagues • Excellent time management skills with a proven ability to meet critical deadlines. • Fundamental Skills with Microsoft Office Suite • Medical Assistant certificate must be active and in good standing • Ability to sit for long periods of time • Ability to speak clearly and listen attentively, often by telephone, for an extended period of time • Ability to understand, process, and take thorough notes in real-time on telephone conversations
