• Provide training, coaching, and technical assistance to community health centers to support outreach programs, quality improvement initiatives, and workforce development • Analyze outreach and engagement data, monitor program performance, and support grant compliance and reporting • Supervise, mentor, and develop Community Outreach Specialists and related staff, including recruitment, onboarding, performance management, and professional development • Collaborate with internal and external partners to develop workflows, policies, and best practices • Support patients with preventive care and chronic condition management through education, care navigation, appointment coordination, and barrier resolution • Identify and address social and logistical barriers to care, connect patients with community resources, and support referral follow-up and documentation • Participate in interdisciplinary care teams, quality improvement activities, and program planning • Ensure compliance with organizational policies, HIPAA requirements, patient safety standards, and grant-funded program expectations
• High school diploma or GED required • Associate degree or relevant certification preferred • Three to five years of experience in community health, care coordination, outreach, population health, or public health programs • Experience leading community health worker activities • Previous supervisory or team leadership experience • Experience working with diverse, underserved, and community-based populations • Basic understanding of medical terminology and healthcare systems • Proficiency with electronic health records, care management platforms, and mobile documentation tools • Strong organizational, communication, and problem-solving skills • Experience writing, organizing, reporting, and presenting data from operations and/or research initiatives • Colorado Department of Public Health & Environment Community Health Worker Registry credential or ability to obtain it within a designated timeframe preferred • Experience performing or supporting basic clinical screening activities, including blood pressure measurement or point-of-care testing preferred • Experience with community organizing, community engagement, or community-based participatory research preferred • Knowledge of existing community resource networks preferred • Bilingual or multilingual proficiency reflective of the communities served preferred
