• Coordinate and facilitate program performance and quality improvement activities • Serve as the CRR point person for Senior Services, AWC, and HCS programs • Coordinate and monitor day-to-day service delivery and program operations within Home Care Services and Adult Wellness Center • Coordinate and monitor intake, case transfer, assignment, and service delivery processes • Maintain accurate and up-to-date participant, private-pay participant, and personnel files • Support IDOA/MCO billing processes, including private pay • Track service utilization, staff coverage, service assignments, supervisory visits, and record keeping • Consult with program staff regarding participant issues and support problem resolution • Provide regular updates, concerns, and efficiency or quality recommendations to the Director • Plan, develop, organize, implement, and direct performance and quality improvement activities • Oversee quarterly internal case record reviews and support PQI activities • Collect, maintain, manage, and analyze participant service delivery, operational, statistical, and program data • Identify service-delivery gaps and recommend improvement plans • Assist with quarterly and year-end dashboards, PQI reports, statistical reports, outcome reports, participant surveys, and satisfaction processes • Analyze trends and innovations in HCS and ADS sectors and make recommendations • Support stakeholder communication and engagement activities • Report incidents according to agency and IDOA procedures • Directly supervise, train, and consult with three HCS Senior Supervisors • Provide support and supervision to HCS Supervisors and other senior services staff when needed • Provide orientation and onboarding training to newly hired staff • Conduct regular supervision and maintain comprehensive supervisory notes • Ensure timely probationary and annual performance evaluations • Participate in staff hiring interviews and provide input to the Director • Participate in staff meetings and professional development training • Foster positive, collaborative, and respectful working relationships • Submit personal timesheets and ensure compliance with agency and funder guidelines • Work under the guidance of the Senior Services Director and exercise independent judgment in daily operations
• Bachelor’s degree in Social Work, health or human services, or related field from an accredited college or university • Prior experience working in the in-home care services field and with older adult populations strongly preferred • Prior supervisory and/or program management experience preferred • Successful completion of CCP management training provided by IDOA or its designee when required • Bilingual in Spanish and English required • Computer literacy required • Proficiency with Microsoft Office applications including Outlook, Word, Publisher, and Excel • College-level math skills required • Ability to accurately prepare and analyze fiscal and statistical reports • Ability to work independently and collaboratively • Must develop and maintain positive and professional relationships with internal and external stakeholders • Must demonstrate tact, professional composure, diplomacy, effective communication, relational, supervisory, time management, organizational, and attention-to-detail skills • Personal cellphone required and ability to respond during working and non-working hours • Must be able to lift and carry up to 25 lbs • Subject to drug and alcohol screening, aptitude and skills testing • Sensitivity to cultural diversity required
