📋 Description

• Support care management activities and assigned care teams • Perform member outreach by telephone or home visits • Discuss care plans, service plans, next steps, resources, questions, and concerns with members • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Support member and provider inquiries and concerns related to care plans and service plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments and screenings and document member care needs • Maintain member records according to state and regulatory requirements • Follow contractual, regulatory, and organizational standards and policies • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education about available benefits and resources • Perform other duties as assigned

🎯 Requirements

• High School diploma or GED • 1–2 years of related experience • Prefer candidates residing in California • Ability to work PST hours preferred • Healthcare experience preferred • Customer service experience preferred • Associate degree preferred
